Gather Your Details: Ensure you have your PIP claim number, insurance company name, and the name and phone number of your claims adjuster.
Download and print the Referral for Auto Accident Treatment form and present it to your evaluating provider. More referral details and a list of qualifying healthcare professionals is included with this form.
Receive an evaluation from a qualified healthcare professional. To use Personal Injury Protection (PIP) in Oregon for massage therapy, you must first be evaluated and referred by a qualified provider. Once your doctor issues the prescription, you may ask them to FAX it to the number provided on our referral form, or you may bring it with you to your massage appointment so we can bill your insurance directly for medically necessary treatments.
Schedule Your Massage: after clicking the “Book Appointment” button below, select the Post Car Accident Recovery service and proceed to schedule an available date and time for your appointment.
Download, print and complete the Auto Accident forms and bring them with you to your massage appointment.
